Caregiver Job Description

  •  Follows care guidelines outlined in the client’s individualized care plan.
  • Performs but is not limited to: personal care assistance, Alzheimer’s/Dementia Care, stroke recovery, ambulation/mobility assistance, transportation needs, meal preparation, and housekeeping.
  • Reports any observed or reported changes in the client's condition and/or needs to the office in a timely manner.
  • Documents care provided and completes the forms required for the client's records.
  • Communicates effectively with all members of the office staff.
  • Maintains confidentiality in all aspects of the job.

Qualifications

  • Valid Georgia CNA license
  • Valid driver’s license, car insurance and reliable transportation
  • Previous experience as a caregiver in a home care setting, assisted living, skilled nursing, or rehab facility, or hospital.
  • Demonstrated ability to read, write, and follow a written plan of care.
  • Must pass a background check, and drug screen..
